RN Triage - HEB Wellness Primary Care
Responsibilities The Triage Nurse will provide professional nursing assessments, prioritize treatments according to the urgency of need, supporting call center staff and patients, ensuring patients receive the right care at the right time. Supervisory Responsibilities: None. Duties/Responsibilities: Interviews patient (if possible) and/or family to obtain medical histories and to make an accurate assessment. Efficiently assigns a triage priority based on observations and available information. Collaborates with our triage APP to appropriately manage triage patients. Assists with scheduling patients as indicated for safe and appropriate and level of care. Initiates diagnostic and therapeutic measures as indicated by using approved guidelines and standing protocols (ie ClearTriage software). Calls for crisis intervention if appropriate. Communicates and acts as liaison with patients, family, and health professionals in a manner that respects confidentiality and dignity. Coordinates the needs of the patient with supporting departments and assists in solving problems related to the welfare of patient and staff. Charts concisely, descriptively, and legibly indicating the status of the patient’s condition, needs, and recommendations provided. progress or lack of progress. Refills medications per institutional approved policies and procedures. Demonstrates compliance with existing standards and policies. Participates in mandatory in-services. Provides education to patients (home-care instructions) Promotes positive patient outcomes Required Skills/Abilities: Must have demonstrated knowledge and skills to provide appropriate care to all age groups. Must be able to make good professional decisions quickly despite stressful atmosphere. Strong customer service skills Strong interpersonal and communication skills (verbal and written) Excellent organizational abilities and documentation skills Ability to handle stressful, and and constantly changing conditions and be capable of making independent nursing decisions Must have adequate typing skills and basic computer knowledge Athena Electronic medical record documentation experience preferred Education and Experience: Graduate of an accredited school of nursing, with a bachelor’s degree preferred. Current state license as a registered nurse. Basic Cardiac Life Support Certification preferred Minimum of 3 years of recent clinical experience in a primary care office, home health, critical care, and/or emergency care setting with specialized triage training preferred.. Familiarity and extensive experience with assessing and interacting with patients on the telephone preferred. Physical Requirements: Ability to sit in front of a computer for extensive periods of time. Good manual and finger dexterity to perform designated duties Ability to stoop, squat, or bend frequently. Corrected vision and hearing within normal range to observe and communicate with patients and professional staff. Exposure to patient elements including communicable disease and bloodborne pathogens.
