RN Transplant Coordinator

Job Summary and Responsibilities

As an RN Transplant Coordinator, you will be a pivotal force in transforming lives, orchestrating the intricate journey of transplant patients from referral to recovery.

Every day, you will expertly manage the full spectrum of transplant coordination: from initiating referrals and evaluating candidates, to leading in-hospital patient care, managing education, facilitating communication with all stakeholders including payers, and maintaining critical data.

To be successful in this role, you will combine advanced clinical expertise with exceptional organizational and communication skills, a meticulous eye for detail, and an unwavering commitment to patient advocacy, driving both individual success and program excellence.

Organizes the clinical programDevelops, implements, reviews, revises and maintains policies, procedures, and standards of care for patient populationDevelops, implements, and maintains ongoing continuous quality improvement activities for the clinical program.Serves as resource to hospital staff regarding patient care.Coordinates the patient evaluation process.Performs initial assessment of patient and family according to protocol for acceptance into program
Job Requirements

MINIMUM

Graduate of accredited school of nursing Three (3) years experience with patients in a related setting or three (3) years experience in an intensive care, acute care, telemetry or medical/ surgical settingWorking knowledge of regulatory compliance and survey processesJoint Commission and CMS standardsWorking knowledge of QAPI process.Strong verbal, written communication skillsProficient computer skillsPerformance improvement tools and methodologies such as PDCA, Six Sigma, Lean, etc.
