About the unit At Rehab and Nursing Center Cedar, our teams specialize in long-term care, fostering deep connections with our patients, their families, and friends. Committed to enhancing the quality of life for our residents, we maintain staffing levels and quality scores that exceed regulatory standards. Our facility, designed with the word "home" in mind, opened its doors in May 2024. It features four beautiful internal courtyards, including one exclusively for team members. Our new community dining room is a vibrant space frequented by residents for meals and activities, complemented by an onsite art room and dialysis den. Scope of Work Assesses, monitors, detects, plans and takes appropriate action to prevent potential physiological complications associated with wound, ostomy or continence. Provides information and instruction to patients and their caregivers. Collaborates with and serves as a resource for other health professionals to promote delivery of quality services and patient education within the institution as well as the community. Participates in projects related to area of Wound, Ostomy and Continence Nursing and developing and evaluating educational materials and programs and policies.
