Clinical Practice & Case Management- Provides safe therapeutic care in a holistic and systematic way. Incorporates differences into the provision of care. Integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum to include patient and family education.Responsible for developing, overseeing and implementing comprehensive case management and treatment plans for patients with cardiac and pulmonary diseases.Utilizes the AIDET (Standard Patient Satisfaction Criteria) to communicate with others, with a focus on patients and their families.Provides monitored exercise therapy to clients in the Lifestyle Management Program (i.e. with the diagnosis of heart disease, COPD, diabetes, PAD, obesity, etc.).Maintains current clinical data, insurance information and program records on each client.Adjusts treatment plan in accordance with established protocols, co-morbidities, client goals/needs and medical history.
