Office Location:   Florence, AL
Coverage area:   Franklin and Colbert counties 

Schedule:   Monday thru Friday, 8am  - 5pm

Responsibilities

 As a RN Case Manager You will:    

Assess/monitor physical, emotional, and psychological needs of patientsCreate hospice care plans that align with the patients wishes and goalsDirect nursing care: administering medications, treatments, and interventionsProvide pain and symptom managementEducate and support the patient’s family and caregiversCollaborate with interdisciplinary teamMaintain accurate and timely documentationParticipate in on-call rotation as required by local branch   Qualifications Graduate of an accredited nursing school with current licensure to practice in state of operationOne year nursing experience required, 3+ years preferredExperience in hospice or a similar setting preferredKnowledge of the hospice philosophy of careCommitment to clinical and documentation excellenceMust be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order  About our Line of Business At Hospice of North Alabama, our primary ambition is to bring first-class care to the warmest and most serene environment possible: home. We provide the full spectrum of hospice services, thoughtfully tailored to the individual needs of patients and families. Hospice of North Alabama, an affiliate of BrightSpring Health Services, is grounded in a foundational commitment to providing exceptional patient care and championing our agency leadership and teams. We are proud to have expert clinicians and caregivers on our team.
