RN re-visit Nurse - Full Time - Eddy SeniorCare (Rotterdam)
Position Summary:
The primary purpose of your job position is to promote and maintain the health of participants enrolled in Eddy Senior Care through teaching, counseling, and appropriate preventive services. The Registered nurse (RN) is responsible for assessment of health needs and implementation of nursing care plans for these participants. The RN provides direct nursing care to the participants, supervises the day-to-day nursing activities including care provided by home health aides. Such supervision must be in accordance with current state, federal and local standards, guidelines, and regulations that govern our facility, and as may be required by the Nurse Manager of Clinical Operations, to ensure that the highest degree of quality care is maintained at all times.
Work Schedule:
10 hour day shift position, 4 days per week (rotating day off per week)
No Holidays, No Weekends, No On Call!
Requirements:

We are a Program of All-Inclusive Care for the Elderly (PACE)--a unique model that is both the Medicare/Medicaid insurer and healthcare provider--giving us maximum ability to customize services to meet participant needs. As an insurer, we are at full risk, with the ability to combine Medicare and Medicaid resources to provide care in creative ways to reduce hospitalizations and avoid/delay nursing home placement. We care for 140 seniors, age 55 yo and above who have needs for long term support in the community. As a healthcare provider, we operate a primary care clinic, rehab clinic, a licensed home care agency, transportation service, adult daycare program, home delivered meals, activities, RN case management, social work, and more! We provide a team-based model of care including a physician, RNs, LPNs, Home Health Aides, social workers, physical/occupation/speech therapists, transportation, a day center team, and more! This is a truly unique program our region is fortunate to have access to, with only 150 PACE programs across the country!
Pay Range: $36.50 - $49.95
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
