The Nightingale RN Case Manager role offers a unique opportunity for an experienced care manager to remain actively engaged in patient care while sharing valuable knowledge and mentorship with newer caregivers across the organization. This position works collaboratively with interprofessional teams throughout the continuum of care to ensure safe, effective and timely transitional care coordination. Grounded in patient advocacy, holistic assessment and individualized care planning, the role focuses on educating patients and families during the discharge process, identifying and addressing barriers to a safe transition home and connecting patients with appropriate resources such as home care or skilled nursing facilities. This is a bedside-based role without direct hands-on nursing tasks, designed to reduce readmissions while maintaining compliance with regulatory and accreditation standards. It is an ideal opportunity for nurses looking to step away from traditional bedside care while staying closely connected to patients and clinical units across the Main Campus.

This is a PRN position. A caregiver in this role will work varying hours between 8:00am and 4:30pm, about 1-2 shifts per week. There are no weekend or holiday requirements.

A caregiver who excels in this role will:

Support care managers in working a difficult case by using their knowledge and expertise to guide a newer care manager.Perform the clerical functions of Care Management to allow the care managers to perform more transitions and follow up.Document Initial Assessments on all new patients and help patients complete POA or Advance Directive forms.

Minimum qualifications for the ideal future caregiver include:

Graduate from an accredited school of professional nursingCurrent state license as a Registered Nurse (RN)Basic Life Support (BLS) certification through the American Heart Association (AHA) or the American Red CrossMinimum of 3 years’ experience in RN Case Management position at the Cleveland Clinic immediately prior to retirementKnowledge of acute care regulatory/accreditation requirementsDischarge planning and/or Quality Improvement experience

Preferred qualifications for the ideal future caregiver include:

Bachelor’s of Science in Nursing (BSN)5-6 years of experiencePrevious bedside experience

Physical Requirements:

Requires full range of motion, manual and finger dexterity and eye-hand coordination.Requires standing and walking for extensive periods of time.Requires normal or corrected hearing and vision to normal range.Sedentary work exerting up to 10 pounds of force occasionally (Occasionally: active or condition exists up to 1/3 of the time) and /or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
