SHIFT/SCHEDULE:  FT Days, Monday-Friday, 8:30 a.m. - 5:00 p.m.

Job Summary

The Nurse Navigator ensures prompt patient care and is the main entry point. They collaborate with the healthcare team to meet patient needs, organize care, and review requirements. The successful applicant will work autonomously and with others to advocate, educate, and coordinate for patients. Good communication skills are required.

Essential Functions of the RoleYou'll coordinate with health professionals to conduct necessary tests for clear diagnoses. Ensure patients have smooth access to top-quality healthcare at all treatment stages.Collaborating with doctors and providers is vital. Education for patients and families is essential. Include diagnoses, treatments, and services. Your role is crucial in patient care.Your role will require an exemplary level of responsibility including decision-making and delegation to ensure safe, professional patient care.Regularly visit physicians and staff. You will be a liaison between them and patients. Provide information about our facilities and general guidance to patients and families.You may need to help patients and families navigate the hospital and provide regular follow-ups.Key Success FactorsCommunicate effectively, both orally and in writingOrganise tasks and work efficiently, as it's vital in evaluating the needs of patients and executing care plans.Navigate through the complexity of the healthcare system, acting as a go-between and advocate for patients throughout their healthcare journey.Belonging Statement

Qualifications

Bachelor degree.You should have at least (3) three years of relevant experience.License Registered Nurse (RN)(BLS) Basic Life Support should be acquired within (30) thirty days of being hired.

PREFERRED EXPERIENCE:

General Surgical and/or Oncology RN experience in an acute care and/or clinic setting with Oncology patients including:  Endrocrine; Breast; Hepatobilary - Liver and Pancreas

Bilingual in Spanish
