Description
The MDS (Minimum Data Set) Coordinator in Training is a registered nurse who is completing a 60-day training program that includes successful completion of the American Association of Post-Acute Care Nursing’s Resident Assessment Coordinator- Certified (RAC-CT) and Resident Assessment Coordinator-Certified Advanced (RAC-CTA) examinations in preparation for filling the facility’s MDS Coordinator/Medicare Specialist position managing the MDS assessment and care planning processes for long-term and short-stay residents.
Training Requirements
Successfully complete the American Association of Post-Acute Care Nursing’s Resident Assessment Coordinator-Certified (RAC-CT) and Resident Assessment Coordinator-Certified Advanced (RAC-CTA) examinations within 60-days of hire.
Demonstrate the ability to perform the essential functions of the MDS Coordinator within 60-days of hire.
MDS Coordinator Essential Functions
Administrative and Leadership Functions
Record all required entries in the MDS in accordance with facility policy and collaborate with the care team to develop and/or update the care plan.
Ensures the MDS complies with regulations as described in the RAI Manual. Timely, accurate, and supported by clinical documentation.
Participates in QAPI activities as appropriate.
Effectively responds to resident and family member complaints and grievances.
Resident Protection from Abuse, Neglect, and Exploitation
Protect residents from abuse, neglect, and exploitation. Immediately report allegations of abuse, neglect, and exploitation to the administrator (Abuse Coordinator).
MDS Coordination
Maintains and follows a schedule of due dates for MDS assessments
Coordinates the timely completion of the MDS by all disciplines.
Participates in scheduled clinical, rehab, restorative, utilization review, QAPI, and care plan meetings as assigned by the facility.
Reviews and audits the MDS data for accuracy and meets with appropriate staff as needed to assure accuracy.
Serves as the coordinator to ensure care team members complete assigned MDS items accurately and timely. Intervenes as needed and communicates delays to the director of nursing and administrator.
Audits the record to ensure supporting clinical documentation supports MDS coding.
Submits the MDS and verifies that the MDS assessments have been transmitted and approved by the State in a timely manner.
Corrects rejected records and prepares them for re-submission.
Modifies MDS if required in accordance with the RAI Manual.
Ensures timely completion of Care Area Assessments (CAAs) according to RAI Manual guidelines.
Participates in formulation of Resident Care Plans (RCP) in collaboration with the interdisciplinary care plan team.
Administers and documents medications and treatments as prescribed by the physician and in accordance with facility policy.
Conducts routine audits and scrubs of MDS to identify/resolve discrepancies, ensure program integrity, and accuracy of quality measures.
Participates in utilization review, Medicare management, chart audits and triple check meetings.
Care Plan and Assessment Functions
Participate in the care planning process in accordance with facility policy based on resident preferences and person-centered care.
Review care plans daily to monitor and identify changes and updates.
Collaborate with the care team and share information to inform them about the completion of the Resident Assessment Instrument (RAI).
Admission, Transfer and Discharge Functions
Greet residents and escort them to their room, provide new resident orientation to the household.
Introduce the new resident to other residents and personnel as appropriate.
Leads and participates in learning circles with residents and associate in the household as applicable
Non-Essential Functions
Staff Development Functions
Attend and participate in facility in-service training programs as assigned including resident rights, prevention of abuse and neglects, behavioral management, and competencies of the CNA.
Attend and participate in scheduled orientation programs and activities.
Attend and participate in scheduled training and educational classes.
Safety Duties and Responsibilities
Administers emergency first aid and CPR when appropriate. Initiates and facilitates the first emergency response in accordance with facility policy.
Follow established infection prevention and control procedures.
Follow established procedures in the use and disposal of personal protective equipment (PPE).
Follow established safety precautions in the performance of all duties.
Report all hazardous conditions and equipment to the director of nursing.
Prepares incident reports and reports safety violations in accordance with facility policy.
Report any communicable or infectious disease to the Director of Nursing and/or Infection Preventionist.
Report missing/illegible labels and safety data sheets (SDSs) to the supervisor.
Report defective equipment to the director of nursing; utilize lockout/tagout as directed.

Experience and QualificationsMinimum of five-year’s experience working as a registered nurse in a post-acute and/or long-term skilled care setting. Experience with MDS 3.0, CAA, and care planning processes a plus. Specific RequirementsMust be able to read, write, speak, and understand the English language.Ability to adjust schedule if needed to meet fluctuating case load demands related to the facility’s daily admission, transfer, and discharge schedule.Experience and comfort using an electronic medical record. Experience with PointClick Care a plus. Ability to effectively provide coaching and in-service training to other team members on RAI process.Must be a supportive team member, contribute to and be an example of teamwork and team concept.Must possess the ability to make independent decisions when circumstances warrant such action.Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the public.Must possess the ability and willingness to collaborate harmoniously with other personnel.Must demonstrate the ability to hold interdisciplinary team members accountable for timely and accurate completion of MDS assignments, CAAs and care plans. Must have patience, tact, cheerful disposition, and enthusiasm as well as the willingness to manage difficult residents and associates.Must be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.Must be able to relate information concerning a resident’s condition.Must not pose a direct threat to the health and safety of other individuals in the workplace.
