**Weekend Track RN HOSPICE Admissions**
The Community Hospice has a rewarding opportunity for a track RN to work as a part of our Admissions Team in Hospice Home Care with cross training for admissions to other levels of care.

**_Workdays only Every_** **_Saturday&Sunday_** **8a-8p -** no hours required during the week once trained

**What you will do:**
+ The Hospice Admissions RN admits patients to hospice services
+ The RN coordinates with the patient's community physician and our hospice medical directors to confirm eligibility for hospice services.
+ Will go to patients' homes including skilled nursing facilities to complete the initial assessment of patient/family needs and begins implementation including ordering medications, medical equipment, supplies and support from our interdisciplinary team which includes social workers, chaplains, volunteers, complementary therapists, home health aides, and more!
+ Ensures a smooth hand-off to the Hospice Case Manager.
+ Corporate office is 445 New Karner Road-Albany NY with satellite offices in Catskill and Saratoga NY. Admissions are across 8 counties: Columbia, Greene, Albany, Rensselaer, Schenectady, Saratoga, Washington. Your region is considered in patient assignment.
**What you will need:**
+ Valid NYS RN license
+ Associate’s degree in nursing. BSN Preferred
+ 1 year of RN experience is required. At least 1 year experience as an RN working in homecare highly preferrable
+ Valid driver license in good standing and reliable vehicle
+ Previous hospice experience preferred but NOT required.
+ Must be able to communicate effectively with patients, physicians, and peers.
+ Must be able to document key information concisely and accurately
+ Strong computer skills
+ Strong organizational and time management skills
Community Hospice provides a thorough orientation, preceptors and a supportive work environment. Training provided in end of life care.

SUPPORTED BY A HOSPICE RN SUPERVISOR in the office 8am to 4pm Saturday and Sunday and an OnCall RN Supervisor after hours, so assistance and guidance is only a phone call away!

Experience based (non-weekend track) Pay Range:$35.00 - $47.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
