RN Hospice Case Manager Float
Overview

**This position is based in Lakeland, FL and requires travel within Polk County territory.  It is full-time, M-Friday, 9am - 6pm.  Compassionate Care Hospice, an Amedisys Company, is seeking a dedicated RN Case Manager Float Nurse to join our exceptional team in Lakeland.  Our team is known for going above and beyond—not only for our patients, but for one another. Salary Range: $80,000–$85,000 annually, with additional bonus opportunities.
Final compensation may vary based on location, job-related knowledge, skills, and experience.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.


Responsibilities

Performs patient assessments and collaborates with the care team to develop and implement a plan of care.Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.Develops, prepares, and maintains individualized patient care progress records with accuracy, timeliness and according to care center policies.Records pain/symptom management changes/outcomes as appropriate.Teaches patients, families, and caregivers appropriate patient care techniques.Regularly communicates with clinical manager and care team.Performs on-call responsibilities and on-call services to patients/families as assigned.Other duties as assigned.
Qualifications

Current RN license, specific to the state(s) you are assigned to work.

1+ year(s) of clinical RN experience in acute healthcare setting(s) - REQUIRED.Current CPR certification.Ready to Take the Next Step?
