**Description**
+ **Registered Nurse – Hospice**
+ **Full-Time | 1.0 FTE | Day Shift**
+ **Salary Range: $48.58 – $75.90/hr (DOE)**
+ **Location: Vancouver Washington**
PeaceHealth is seeking a compassionate **Hospice RN** to join our dedicated team. This is a full-time, day-shift opportunity to provide meaningful, patient-centered care to individuals and families navigating end-of-life journeys.
**What You’ll Do:**
+ Perform initial patient and home evaluations to determine eligibility and safety for hospice services.
+ Deliver skilled nursing care, education, and support to patients and caregivers.
+ Develop and manage individualized care plans in collaboration with the patient’s physician.
+ Provide oversight and direction for Home Health Aide care plans.
+ Partner with the interdisciplinary team to coordinate care and ensure seamless services.
+ Lead discharge planning and assess outcomes to align with patient goals.
+ Document care accurately and timely in compliance with all regulatory and agency guidelines.
+ Support families with bereavement services following patient death.
**Qualifications:**
+ Graduate of an accredited School of Nursing; BSN preferred.
+ Current RN license in the state of practice.
+ Current Basic Life Support (BLS) certification.
+ Valid driver’s license and proof of auto insurance.
+ Minimum 1 year of nursing experience in hospice, home health, or end-of-life care required (hospice experience preferred).
+ Strong clinical judgment, compassion, and the ability to adapt care to meet the needs of patients across the lifespan.
