**Description** **Hospice RN – Full Time (Day Shift)** **Location:** Vancouver, WA **FTE:** 1.00 | **Schedule:** Day Shift **Hourly Rate:** $48.58 – $75.90 (DOE, union-represented) **Hiring bonus and relocation assistance may be available.** **The work schedule for this position is;** + **Week 1 - 5 days which includes 1 weekend day (8 hour shifts).** + **Week 2 - 4 week days (10 hour shifts).** Join PeaceHealth’s mission-driven team in Vancouver, WA, and provide compassionate end-of-life care as a **Hospice RN** . This full-time position offers the opportunity to make a meaningful difference in the lives of patients and families during their most vulnerable times. As a Hospice RN, you’ll deliver skilled nursing care to patients in their homes, guiding them through personalized care plans focused on comfort, dignity, and quality of life. You’ll collaborate closely with an interdisciplinary team to provide physical, emotional, and spiritual support—ensuring seamless, patient-centered care. **Key Responsibilities:** + Complete initial in-home evaluations and determine eligibility for hospice services + Develop and manage individualized care plans in collaboration with the physician and care team + Provide education to patients and caregivers on treatment, medications, and symptom management + Coordinate services with Home Health Aides, therapists, chaplains, and social workers + Monitor patient conditions, adjust care plans, and document all services accurately and timely + Support bereavement services and participate in discharge planning **Qualifications:** + RN license in Washington (required) + Basic Life Support (BLS) certification (required upon hire) + Valid driver’s license and auto insurance (required upon hire) + Minimum 1 year of hospice or end-of-life nursing experience (required) + Bachelor of Science in Nursing (preferred) + Strong assessment, communication, and care coordination skills If you’re a compassionate RN with a heart for hospice and a commitment to holistic care, we invite you to bring your skills and empathy to PeaceHealth.
