The Home Infusion RN plays a critical, highly patient-facing role in supporting patients as they transition from hospital-based care to receiving infusion therapy at home. Serving as a clinical and technical subject matter expert, this caregiver works closely with patients, families, hospital teams, and referral partners to provide bedside education, build strong stakeholder relationships, and ensure seamless transitions of care. The role blends hospital-based referral support with home- and virtual-based clinical responsibilities, including infusion equipment education, real-time troubleshooting, and ongoing support for patients, caregivers, and clinical partners, all while advancing awareness and confidence in the home infusion model of care.
A caregiver in this role works Monday through Friday, 9:00 AM – 5:30 PM, with a weekend rotation of one weekend every eight weeks. Saturday coverage is 8:30 AM – 5:00 PM at the Business Operations Center. This role also includes on-call coverage from Monday evening through following Monday morning, and a holiday rotation of one holiday per year. Caregivers must be able to float between Main Campus, the Business Operations Center, and additional facilities as needed.
A caregiver who excels in this roll will:
Provide appropriate training, education and bed-side infusion pump connection services to home infusion patients to facilitate timely discharge from CCF hospitals, outpatient and long-term care facilities. Document in a timely, thorough and accurate manner in the electronic health record.Assist Cleveland Clinic home care nurses and patients initiate and troubleshoot devices and catheter related problems .Provide on-site training and education to contracted agency nurses on IV care and pump usage. Provide daily telephone support / continued education to agency nurses as neededWork closely with Nursing Education to create tools for home care nurses around IV pumps and supplies.Develop interactive teaching methods for home infusion patients to promote safe, independent care.Facilitate the development of infusion policies and procedures for Cleveland Clinic Home Care Services that reflect current, evidence-based guidelines to ensure patient safety.Continuously evaluate infusion products, supplies and protocols to be used in the care of home infusion patients.Create and maintain relationships with case managers, nurse managers and staff from Cleveland Clinic Health System and contractual clients to support the mission of Cleveland Clinic Home Care Services and patients we serve.Actively participate in national nursing, nutrition or infusion organizations advancing the care of home infusion patients.Lead company performance improvement activities including infection control. OR Supports data collection for quality improvement and adherence to regulatory requirements.Participate in Cleveland Clinic committees as well as competency enhancing activities tied to company goals and objectives.Participate in weekend, holiday and on-call rotations when assigned.Minimum qualifications for the ideal future caregiver include:
Graduate of an approved school of Professional NursingCurrent licensure as a Registered Nurse (RN) in the state of Ohio requiredCurrent Basic Life Support (BLS) through American Heart Association (AHA) or American Red CrossRecent nursing experience with strong IV skillsPreferred qualifications for the ideal future caregiver include:
