Job Summary and Responsibilities
Under supervision, the Case Manager plans, organizes and manages a patient caseload: applies concepts derived from management and communications theory, group dynamics, counseling methodologies, and clinical nursing judgment to implement a clinical nursing program or programs. Provides direct patient care through use of nursing process, performs established nursing intervention utilizing current clinical knowledge. Initiates and participates in patient, family and staff education. Demonstrates leadership capabilities. Serves as preceptor for orientees and students.
As our Case Manager RN, you will coordinate the care and services of patients across the continuum of illness; promote effective utilization; monitor health care resources; and assume a leadership role with the interdisciplinary team to achieve optimal outcomes through the tools of case management.
Every day you will perform a wide variety of high-level patient care activities to include admission assessment, screen for appropriateness, and development of a discharge plan. Timely and appropriate discharges will be accomplished by evaluating the patient's post-discharge needs and the provision of services necessary to meet those needs.
To be successful in this role you must be knowledgeable in community resources and alternate care facilities.
