**Description** **Registered Nurse – Home Health** **Per Diem/Relief | 0.0 FTE | Day Shift** **Location: Vancouver, Washington** **Salary Range:** $48.58 – $75.90/hr (DOE) PeaceHealth is seeking a **Home Health RN** to join our compassionate and skilled team in Vancouver, WA. This per diem role offers flexibility while allowing you to make a meaningful difference by providing high-quality, patient-centered care in the comfort of patients’ homes. **What You’ll Do:** + Conduct initial patient/home evaluations and determine eligibility for home health services. + Provide skilled nursing care, education, and support to patients and caregivers according to the plan of treatment. + Perform physical assessments and develop, coordinate, and adjust individualized care plans with the primary physician. + Oversee and clinically manage Home Health Aide care plans. + Collaborate with an interdisciplinary team to ensure seamless care delivery. + Lead discharge planning and evaluate outcomes to meet patient goals. + Document all care timely and accurately in compliance with agency and regulatory standards. **Qualifications:** + Graduate of an accredited School of Nursing; BSN preferred. + Current RN license in Washington state. + Current Basic Life Support (BLS) certification. + Valid driver’s license and proof of auto insurance. + Minimum 2 years of acute clinical or home health/hospice nursing experience required. + At least 1 year of home health experience preferred (or willingness to orient to competency). + Strong clinical judgment, adaptability, and commitment to patient-centered care. **Why Join PeaceHealth?** At PeaceHealth, we are committed to caring for the whole person—mind, body, and spirit.
