Home Base is currently seeking a full-time Registered Nurse to provide compassionate, patient-centered care to veterans, service members and families dealing with the invisible wounds related to Military Service. This role will work collaboratively with our interdisciplinary clinical team to provide care to patients across all intensive clinical services at Home Base, with a primary focus on the Brain Health Program.

Job Summary
SummaryThis position will collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team. They work collaboratively with all healthcare team members to develop and implement treatment plans that support patient-centered plans of care for both individual patients and the medical community.

Essential Functions
Responsibilities will include managing the day-to-day medical needs of patients in the intensive programs, such as taking vitals, drawing labs, administering breathalyzers, providing medical condition management and triage, and collaborating with the treatment team in shared medical case management. In addition, this position will complete medical record reviews of potential patients to determine appropriateness to engage in programming.

Education
Bachelor of Science in Nursing from an accredited program, required.

License and Credentials
Current Massachusetts licensure as Registered Nurse, required.
Experience
-Minimum of 2 years’ experience as a Registered Nurse, required.
-BLS-certified, required.
Knowledge, Skills, Abilities
-Psychiatric nursing experience or knowledge of Veteran/Military population preferred.
-Clinical knowledge of mental health to enable effective interactions and communications with referring clinical providers.
-Clinical knowledge of common medical complaints to support accurate assessment, triage, and appropriate care recommendations.
- Ability to maintain confidentiality and secure sensitive information.
- Knowledge of medical terminology.
- Ability to accurately screen and triage acute patients.
-Exceptional organizational skills and ability to prioritize effectively and complete tasks.
-Highly motivated, independent, and capable of performing in a busy, complex clinical environment.
-Exceptional computer skills (including operating systems, word processing, database, electronic mail, Internet, spreadsheets, and other office management systems such as MS Outlook, MS Office, EPIC, Sales Force).
-Excellent interpersonal, written, and oral communication, and public speaking skills while conversing with patients, families, health professionals.
-Proficiency in phlebotomy required.
