Transplant Coordinators coordinate all aspects of care for both transplant recipients and donors. A coordinator participates in medical, administrative, research and teaching duties. The coordinator is an active member of the multi-disciplinary transplant team.
This RN will be working with the Heart Failure and Advanced Heart Failure team in addition to working with LVAD and heart transplant patients
Development and implementation of IPD critical Pathways
IPD discharge planning
Facilitating and managing the use of home care
Triage of OPD and IPD medical issues with referral to appropriate health care professionals
Communications with insurance companies and referring physicians
Quality assurance projects
Promoting health maintenance education and follow-up
Donor & Recipient Family and patient management
Location: Detroit
Hours: 630AM-5PM, 4- 10-hour shifts. No On Call No Weekends

EDUCATION/EXPERIENCE REQUIRED:
Bachelor's degree in nursing preferred.
Organizational and Interpersonal skills to effectively communicate with patient care personnel and relate to the patients and their families.
Minimum of two (2) years clinical nursing experience in an inpatient or outpatient setting.
Analytical ability and motivation necessary to effectively plan, deliver and implement plans of care to in acute and outpatient situations.
For those working as a Coordinator of Heart Transplant/with VADS: Experience in ICU or OR preferred.
CERTIFICATIONS/LICENSURES REQUIRED:
Current RN license in the State of Michigan.
