RN Coordinator- Advanced Lung Disease

Description:The organ transplant coordinator assists in the care of the patient from candidate referral, evaluation, acceptance, transplant and follow-up. The coordinator is an expert in not only the care of the transplanted patient, but in advanced disease and works within a multidisciplinary team in the management of the patient in both the inpatient and outpatient setting. Included in the responsibility is outpatient clinic, telephone triage, notifying patients of lab and test results, and medication changes. Education for staff and patients, both hospitalized and in the ambulatory setting, is essential. The coordinator will also be well versed in policies related to donation and transplant.
The transplant coordinator will be working with pulmonary, advanced lung disease patients.

III. Position Requirements:

Minimum Education: Required: Bachelor’s Degree

Specify Degree(s): Nursing

Minimum Experience:

Required: 1-2 years of previous job-related experience

Preferred: 3-5 years of previous job-related experience

Licensure/Certifications:

Required:

CPR (Cardiopulmonary Resuscitation)

Current Registered Nurse License State of Illinois

Preferred:

Specialty certification in clinical or functional area of nursing granted by a national nursing organization.

Other: National Certification as Transplant Coordinator or other related specialty preferred.
