GENERAL SUMMARY:
· Reports to the Nursing Leader/Administration. The Nurse Coordinator is a Registered Nurse who under the direction of department leadership develops, implements, monitors and evaluates a program of care for patients.
· Coordinates care across the continuum to facilitate quality outcomes.
· Collaborates with other healthcare team members including physicians, nurses, extenders, ancillary and support staff to ensure optimum patient care and satisfaction.
· Provides education to patients and family regarding disease process. Trains and directs a nursing team in the optimal care of the patient.
EDUCATION/EXPERIENCE REQUIRED:
· Graduate from an accredited school of nursing.
· Successful completion of HFHS (Parent Operating Group) Pharmacology test.
· Maintains and demonstrates current clinical knowledge in area of specialty. Bachelor's Degree in Nursing preferred.
· A minimum of three to five years of progressively more responsible clinical nursing practice in specialty.
· Certification in specialty preferred.
· Clinical knowledge and analytical abilities necessary to formulate effective nursing care plans.
· Ability to perform a wide variety of professional nursing services.
· Ability to effectively communicate with patients, families, and medical group personnel.
· Preferred experience in the specialty.
· Basic computer knowledge and keyboarding skills. Knowledge of Word and Excel.
CERTIFICATIONS/LICENSURES REQUIRED:
· Current licensure by the Michigan State Board of Nursing as a Registered Nurse. Current BLS or ACLS per HFHS Policy.
