Aspirus Health in Wausau, WI is seeking a RN – Comprehensive Care Coordinator to join our team!
The RN Comprehensive Care Coordinator is a Registered Nurse (RN) directly responsible for assessing, coordinating, and integrating healthcare for patients with complex or chronic conditions across the continuum of care. This RN position manages all aspects of patient centric care coordination through collaboration with physicians, staff, and other health professionals to assist patients to reach optimal function and health in an appropriate environment that supports their needs. The RN Comprehensive Care Coordinator will work directly with the entire health care team to deliver innovative population health management practices for patients with ongoing medical and/or social concerns leading to higher utilization of health care services.
HOURS: Full-Time or 1.0 FTE, 80 hours every pay period. Day shift.
Experience/Qualifications
Knowledge of utilization review standards and patient care practices normally acquired through graduation of an accredited school of professional nursing.A minimum of five years of acute care hospital experience.Experience in critical care, general medical and/or surgical nursing, and experience in educational presentations beneficial, as well as previous experience in the application of Utilization Review activities and medical record reviews.Possesses an active Wisconsin RN nursing license. Will need to pursue a Michigan nursing license.A bachelor’s degree in nursing, successful completion of quality or Utilization Review certification examination, and active membership in professional organizations (i.e., NAHQ, WAHO) beneficial.
