The main responsibility for this position is to reconcile the medication list that a patient is taking from the referral and discharge summary. The RN will review the medication list that is created by a Physical Therapist after an admission is performed. The nurse will contact the patient /caregiver and the provider/pharmacy with any discrepancies or findings. The nurse will review risks, and adverse reactions of medications and educate patient/caregiver over phone. Requirements: + NYS RN License + Valid NYS Driver's License + Experience in geriatric diagnosis and medications + One year of pervious experience in the health care field + Must be knowledgeable in medications and be able to teach over phone + Must be able to communicate with the Physical Therapist + Must be able to multitask, including tracking and following through on medication changes + Must have good attention to details and processes Preferred: + EPIC background a plus + Knowledge of regulations for a certified home health agency a plus This is a rewarding opportunity to prevent hospitalizations and educate patients! Pay Range: $33.10 - $47.15 _Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates.
