Connecticutrsquos most comprehensive healthcare network as an RN Case ManagerFor over years St Vincentrsquos Special Needs Services SVSNS has been a trusted provider of specialized services for children and adults with multiple developmental disabilities and complex medical needs such as cerebral palsy acute brain injury neuromuscular disorders and autism spectrum disorders Each year more than children adults and their families receive services from the organizationProvide comprehensive nursing oversight in adult residential group homes for medically complex individuals The Nurse Case Manager ensures optimal care and safety for residents manages health assessments and supports staff through training and audits The role involves collaboration with interdisciplinary teams to coordinate care plans monitor outcomes and maintain compliance with regulatory and quality standardsThe Nurse Case Manager participates in a collaborative coverage model overseeing nursing needs across multiple group homes rather than a single site Each nurse may cover up to homes dedicating approximately ndash hours per home weekly This structure supports individualized care and continuity while aligning with HHCrsquos standards for regulatory compliance and quality improvementClinical Oversight Documentationbull Reviews all documentation by staff including unlicensed staff and LPNsbull Audits Medication Administration Records MARs physician orders and medication practices in partnership with HHC Qualitybull Ensures completion and follow up of medical appointmentsbull Coordinates directly with providers when neededbull Completes nursing notes per DDS and HHC Quality recommendationsbull Conducts quarterly medication review auditsbull Serves as primary medical contact for state or regulatory inquiries related to assigned residents clinical practices or nursing oversight within designated homesResident Assessments Care Planningmiddot Performs nursing assessments on admission weekly and with any change in conditionmiddot Develops reviews and updates nursing care plans quarterly and as neededmiddot Participates in new admission planning includingo Nursing Health and Safety Assessmento Self Medication Assessmento Fall Risk Assessmento Health History and Diagnoseso Care Plan developmento Completion of resident specific trainingParticipates in all Individual Plans IPs for
