The campus features a park-like setting with landscaping and a walking path that winds through the grounds and connects to Millennium Park. Care Coordination consist of Case Management, Social Services and Utilization Review. The personnel in this office are responsible for discharge planning, assuring proper patient billing, assuring compliance with all payors requirements and regulations. **Preferred Qualifications** **Role Purpose** Coordinates, negotiates, procures and manages the care of patients to facilitate achievement of quality cost-effective outcomes. Works collaboratively with physicians and interdisciplinary staff, internal and external to the organization, to identify and resolve issues.
