RN Case Manager
Full-Time RN Case Manager Location: In-home assessments between Framingham and Canton, MA Schedule: Monday–Friday | 9:00 a.m.–5:00 p.m. | 40 hours/week Assignment Type: Temp-to-Perm or Direct Placement Compensation + Contract Rate: $57/hour + Direct Hire Salary: $100,000/year + Mileage Reimbursement: $0.45/mile Position Overview Amergis Healthcare Staffing is partnering with a respected Home Health organization that supports older adults and individuals with disabilities to remain safe, independent, and well-supported at home. The RN Case Manager plays a key role in delivering high-quality, community-focused care through individualized care plans, clinical oversight, patient and family education, and comprehensive case management. The Registered Nurse plans, organizes, and delivers home care services, drawing on community resources to meet the needs of individuals and families in their homes. Essential ResponsibilitiesClinical Care + Complete initial patient assessments and full physical evaluations. + Conduct ongoing re-evaluations of patient needs. + Develop, initiate, and revise care plans as patient status evolves. + Utilize assessment data to establish nursing diagnoses. + Create goal-driven care plans involving the patient and family. + Implement preventive and rehabilitative nursing procedures. + Administer medications and treatments as prescribed. + Provide patient and family counseling on health and care needs. + Deliver health education in line with the care plan. + Identify and facilitate discharge planning. + Serve as Case Manager for assigned caseload as designated by the Clinical Supervisor. Communication + Prepare accurate clinical notes and update the primary physician as needed (minimum every 60 days). + Communicate changes in patient condition and obtain physician orders. + Coordinate care with physicians and community health resources. Additional Duties + Participate in on-call rotation per policy. + Coordinate equipment and service needs for patients. + Instruct, supervise, and evaluate home health aide performance every two weeks. Qualifications + Graduate of an accredited nursing program. + 1–2 years of recent acute care experience in an institutional setting. + Active state RN license and current CPR certification. + Bachelor’s degree preferred; 1 year of home health experience preferred. + Strong clinical, communication, problem-solving, and documentation skills. + Ability to perform physical tasks including lifting, positioning, and transferring patients. + Valid driver’s license and insured, reliable vehicle meeting state/organization requirements. + Supervisory experience not required; role includes oversight of home health aides.
