RN Case Manager

The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.
Minimum Requirements:
+ Current RN licensure in state practicing
+ At least one year of Case Management experience preferred
+ Current CPR if applicable
+ TB questionnaire, PPD or chest x-ray if applicable
+ Current Health certificate (per contract or stateregulation)
+ Must meet all federal, state and local requirements
+ Must be at least 18 years of age
Job Summary:
We are seeking a dedicated and detail-oriented Registered Nurse (RN) CaseManager to join our California Children’s Services (CCS) team in Oxnard,CA. This is a fully office-based role focused on telephonic case management andmedical eligibility review for pediatric patients enrolled in the CCS program.
Key Responsibilities:
+ Conduct telephonic case management for children enrolled in the CCS program.
+ Perform medical eligibility reviews by analyzing medical records and documentation.
+ Compose detailed case notes and summaries with a high volume of typing.
+ Communicate effectively with families, providers, and other stakeholders via phone.
+ Apply clinical judgment to assess and coordinate appropriate services.
+ Collaborate with interdisciplinary teams to ensure continuity of care.
REQUIRED:
+ At least 1 year of experience with medical eligibility review as an RN.
+ Bilingual preferred; not required
