The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime. This is a fully office-based role focused on telephonic case management andmedical eligibility review for pediatric patients enrolled in the CCS program. Key Responsibilities: + Conduct telephonic case management for children enrolled in the CCS program. + Perform medical eligibility reviews by analyzing medical records and documentation. + Compose detailed case notes and summaries with a high volume of typing. + Communicate effectively with families, providers, and other stakeholders via phone. + Apply clinical judgment to assess and coordinate appropriate services. + Collaborate with interdisciplinary teams to ensure continuity of care.
