RN Case Manager
Position: Nurse Manager - Clinical Resources/Case Management Location: Hyannis, MA Facility Type: Hospital Start: 12/29 Duration: 13 weeks Hours: 40 Shifts: + M-F Days Rotating on call for weekends & holidays Requirements: + Permanent MA RN license + 5+ years Case Management experience + Managerial/Supervisory experience + - EPIC experience required - Permanent MA RN license in hand - Minimum 5 years Case Management experience - 2 years of experience in process improvement - Knowledge of CMS regulations - Management/Supervisory experience strongly preferred The RN Case Manager is responsible for coordinatingcontinuum of care activities for assigned patients and ensuring optimumutilization of resources, service delivery, and compliance with medical regime.
