**Position Purpose**
As an **RN Care Manager** at St. Mary’s Medical Center, you will be responsible for coordinating and facilitating all aspects of discharge planning for an assigned patient population. You will work closely with physicians, nurses, social workers, and other healthcare professionals to ensure safe and effective transitions of care, while tailoring your approach to meet the age-specific and cultural needs of each patient.
**What You Will Do**
+ Conduct face-to-face assessments within one (1) business day of patient arrival, and no later than two (2) business days.
+ Reassess discharge planning needs daily and document findings in the clinical documentation system.
+ Incorporate cultural, age-appropriate, family, and environmental factors into assessments and patient education.
+ Advocate for continued authorization of post-acute services and obtain prior authorizations for medications as needed.
+ Refer complex cases to the Team Lead and collaborate with physicians on discharge planning.
+ Coordinate with Social Work for patients requiring complex discharge support.
+ Serve as a resource to nursing staff, patients, families, and physicians regarding discharge planning and care transitions.
+ Perform other duties as assigned.
**Minimum Qualifications**
+ **Licensure:** Active **PA RN License** required.
+ **Education:** Bachelor of Science in Nursing ( **BSN** ) required.
+ **Experience:** 2–4 years of relevant nursing experience, preferably in care coordination or discharge planning.
