Responsibilities:
The responsibility and accountability to handle all referrals within the skilled nursing facilities
Manages those coming into the building in the sub-acute unit
Manage referrals via multiple communication platforms (fax, phone and online)
Collaborate and communicate with nursing professionals as well as insurance companies to manage insurance verifications and prior authorizations
Continual review and authorization of discharge planning, ordering supplies and setting up referrals, etc
Manage a strong relationship and partnership with Nursing Leadership
Assist on the unit ass needed
What you will need:
Required:
A current license to practice as a Registered Nurse (RN) in the State of New York
Associate’s degree in nursing or Degree of Nursing from an accredited school
3 years or more working as a RN ideally in long term care
Preferred:
1 year of experience or more in a long term care facility or home care as a RN
Prior experience in case management
Bachelor's degree of nursing
Highest level of customer service while completing day to day duties and responsibilities
Excellent communication in a fast pace work environment across teams on different shifts (RNs different facilities and department) dieticians, insurance companies
Work independently as well as have strong engagement within a team setting to collaborate with others in the facility as well as externally
Manage multiple tasks and can prioritize the most important tasks
Computer literate and can learn multiple different computer applications and how they relate to each other
Critical thinking and can think on their feet
Passion to learn, grow and develop in the nursing field
Pay Range: $39.30- $50.75
Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
