Job Description
Join our team as a Care Manager (RN) where you will assess, plan, and implement medical and support services for post-discharge Medicaid members across the continuum of care. This role is crucial in promoting high-quality, cost-effective care through pre-admission and post-discharge medication reconciliation, while collaborating with care management and coordination teams to identify complex care transition needs. You will ensure members receive appropriate follow-up support after acute care visits and conduct outreach during evenings and weekends to improve continuity of care. Additionally, you will support a team culture focused on achieving daily goals and connecting members to essential Medicaid services.
ResponsibilitiesAssess, plan, and implement medical and support services for Medicaid members post-discharge.Promote high-quality, cost-effective care through medication reconciliation and collaboration with care teams.Ensure appropriate follow-up support for members after acute care visits.Conduct outreach during evenings and weekends to improve continuity of care.Support team culture focused on achieving daily goals and connecting members to essential services.Essential SkillsStrong communication skills with the ability to engage empathetically and clearly.Knowledge of behavioral health resources and how to effectively connect members.Ability to manage multiple technology systems for documentation and outreach.Additional Skills & QualificationsMaster’s Degree in Behavioral Health or Registered Nurse with extensive psychiatric experience.Licensure: LMFT, LMHC, LPC, or RN with extensive psychiatric experience.Must hold a valid Nebraska license.Minimum 2 years in a behavioral health or care management role.Work Environment
This is a 100% remote position with a schedule from Thursday to Monday, flexible between 2 PM and 8 PM, and requires work during evenings and weekends (8 hours between 8 AM and 8 PM). You will have Tuesdays and Wednesdays off. The team is small and highly communicative, allowing for a high level of function and support.
Pay and BenefitsThe pay range for this position is $36.00 - $42.00/hr.
