Expected starting wage range is $52.36 - $64.68. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.
POSITION SUMMARY
The RN Admissions Planner reviews referral information per prescribed timelines, gathers appropriate medical and psychiatric information as necessary, presenting the referral to the psychiatrist, medical director or designee for admission orders. The RN Admissions Planner engages in effective communication with referral sources, program staff, and providers, and coordinates admissions to the program in an efficient manner.
QUALIFICATIONS
Required:
· RN license in state of operation
· Two (2) years of inpatient experience in the treatment of individuals with severe mental illness
· Three (3) months of experience screening psychiatric referrals
· Ability to communicate effectively with a multi-disciplinary team of diverse stakeholders
· Experience with Screening, Brief Intervention, and Referral to Treatment (SBIRT) tools or comparable models of communicating medical issues with providers
· Experience utilizing electronic health records
· Experience with Microsoft Teams, Zoom, or other similar platforms
· Must be at least 18 years of age
· Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
· All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. If the position requires driving, valid driver license, a motor vehicle clearance and proof of auto insurance is required at time of employment and must be maintained throughout employment. ESSENTIAL FUNCTIONS
· Demonstrate the Telecare mission, purpose, values and beliefs in everyday language and contact with the internal and external stakeholders
· Assumes responsibility for admission coordination, including:
o Receiving and screening all referrals and determining appropriateness for admission, following the Telecare policy and procedure of admission and exclusion criteria for designated program
o Follows prescribed timelines for managing referrals including initial response within one hour and prompt replies to questions from program staff or referral sources
o Completes referral log or designated form that details specifics around referral, admission and any delay or denial that is utilized for data tracking and compliance purposes
o Developing and maintaining a professional relationship with the County contract monitor, specialized services, and all referral sources
· If employed at Inpatient program, must assist with restraint of members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
· If employed at Outpatient program, must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training

SKILLS
· Excellent communication skills
· Advanced skills using Electronic Health Record software, Microsoft Suite or 365 and platform communication tools (Zoom, TEAMS)
PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
The employee is occasionally required to bend and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, stand, twist, reach and do simple grasping occasionally. The position requires manual deviation, repetition and dexterity and to occasionally drive and be exposed to uneven walking ground as well as Hazardous Exposure (Blood Borne Pathogens, Hospital Waste, Chemical & Infectious). Visual requirements include computers and books exposure.
