You'll work under the supervision of our Director of Nursing, collaborating to plan, oversee, and evaluate patient care. **As an RN in Long Term Care, You Will:** + **Lead Patient Care:** Utilize the full Nursing Process (assessment, planning, implementation, and evaluation) to develop and implement individualized care plans for residents. + **Assess & Observe:** Collect, record, and report critical assessment data, identifying changes in health status to establish nursing diagnoses. + **Collaborate & Delegate:** Work seamlessly as a team member, effectively delegating components of nursing care to other healthcare team members and reporting intervention status. + **Document & Educate:** Meticulously document nursing interventions and resident responses, and actively participate in teaching plans to empower residents and families. + **Uphold Standards:** Maintain adherence to the State Nurse Practice Act, facility policies, and our mission, demonstrating proficiency in all unit skills. Graduate Nurse (G.
