Position Overview – Home Dialysis Nurse
We are seeking a dedicated and compassionate nurse to join our Home Dialysis team. This role is responsible for the coordination of care, patient assessment, education, and various other nursing interventions for patients on home dialysis. Our program includes both Peritoneal Dialysis (PD) and Home Hemodialysis (HHD).

Schedule: Full-time position consisting of four 9-hour shifts per week during clinic hours.

Key Responsibilities:

Educate and train patients and caregivers on performing dialysis treatments safely and effectively at home

Conduct home visits as needed (mileage reimbursed or company vehicle provided)

Provide ongoing support and care coordination for home dialysis patients

Participate in a rotating on-call schedule to provide remote support for patients after hours

Collaborate with physicians and inter-professional team members to plan, implement, and evaluate individualized dialysis care plans

Document patient history, symptoms, medications, and care provided in compliance with standards and regulations

Utilize technology to enhance care quality and delegate appropriately while maintaining nursing accountability

Qualifications:

Dialysis experience is preferred, though we are open to training the right candidate with a strong clinical background and a passion for patient education

Must practice within the full scope of nursing knowledge and expertise and follow all age- and population-specific care standards

Adheres to the Sanford Professional Nursing Practice framework, including the ANA Scope and Standards of Practice and Code of Ethics

The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide individualized nursing care to dialysis patients.

Job Description

Responsible for the coordination of care, patient assessment, patient education, and various other nursing interventions. Collaborates with other inter-professional colleagues, including physicians, to plan, implement and evaluate dialysis care. Demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned dialysis clinical setting. Functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Sanford Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct. Documents patient history, symptoms, medications and care provided in compliance with applicable standards/regulations.

Utilizes technology to enhance quality patient care, to collaborate with physicians and inter-professional team members, and effectively delegates to and supervise unlicensed personnel, assume overall nursing accountability for patient care.

Qualifications

Bachelor’s Degree in nursing preferred. Graduate from an accredited nursing program required, including, but not limited to, American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).



Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific licensure, competencies and certifications.
