the lives of their clients and their clientsrsquo familiesClinical Supervisor HospiceSupervises the program to ensure that a high quality cost effective service is delivered by the department and that is in keeping with Hartford HealthCare at Homes strategic plan as well as the Programrsquos goals and objectives Provides direct patient care that is specific to each patientrsquos needs including those needs that are age specificIn general most of the time will be spent in the following activitiesProvides staff guidance in making clinical and case management decisions focused on clinical excellence for end of life care and symptom management Directs staff in the Medicare Hospice Benefit guidelinesAssures the development of personnel qualifications and policiesIdentifies and facilitates professional development needs and competency of staff through home visits with staff to supervise and evaluate clinical performance identifies and follows through with development planMaintains compliance with regulatory agency policies and procedures by providing staff guidance in making clinical and case management decisions that are cost effective and focused on clinical outcomesAccountable for team performance in achieving desired clinical and operational performance measuresUtilizes clinical software to identify opportunities for education most appropriate utilization of services oversee clinical progression of patients and trend team outcomes and performanceAssures that clinical and functional outcomes are achieved at the highest level possibleResolves patient care problems collaboratively with internalexternal customers ndash collaborates with peers across the regionsOversees the daily patient assignments for clinical staff based on client needs and staff competency to maintain team productivity standards LEAN Daily Management process Visual BoardsCoordinates referrals and ensures timeliness of SOCs and timeliness of adjunct service deliveryEnsures that patient needs are continually assessedEnsures that an individualized plan of care is developed implemented and updated as needed for all patientsEnsures coordination of patient care including communication with all physicians involved in the plan of careDetermines staffing needs and coordinates hiring to meet service demands for assigned geographic areasHires trains coaches and evaluates staff This role is also responsible for completion of personnel evaluations according to agency timelinesReviews and analyzes team and patient metrics to consistent with agency initiativesParticipates in preceptor programsAnticipates customer needs and responds quickly accurately and pleasantlyParticipates in preceptor programs and student education regarding hospice and
