The Rehab Liaison coordinates the placement of all admissions and transfers in accordance with standards and guidelines. This position works in conjunction with physicians, department leadership, and case management to prioritize admissions to the most appropriate bed available. Criteria for admission is based on medical and functional needs as well as unit specific admission criteria. Liaison is responsible for communicating with all departments to maximize efficiency and expedite the admission and transfer processes.
If applicable, is responsible for marketing Hospital services and acting as a liaison between Hospital departments and current and/or future clients. Provides customer services to physicians, their patients, office administrators and their staff and Hospital clients including nursing homes and other facilities. Maintains active and satisfactory working relationships with Hospital departments, committees and medical staff. Assists with administrative projects as assigned.
Full Time Position, Hybrid after probationary period. Hours are 8:00am-4:30pm with on call required, 1 weekend every 2-3 weeks. Hybrid after probational period.
