Responsible for the delivery of resident care through the process of assessment, planning, implementation and evaluation. Maintains established departmental policies and procedures, objectives, quality improvement program, safety, environmental and infection control standards. Maintains confidentiality of all Nursing Home/staff information. Adheres to Facility Standards of Conduct and practices the Core Values of Service of the Poor, Reverence, Integrity, Wisdom, Creativity and Dedication. Represents OLP accurately and honestly. Acts with integrity and in the best interest of OLP. Identifies instances of non-compliance and takes appropriate action, including reporting violations of the CRP. Adheres to Facility policy or regulations. Business relationships are conducted in the best interest of OLP, free of gifts, favors or other improper inducements. Position Accountabilities and Performance Criteria. The following are essential job accountabilities: 1) Under the supervision of the Unit Manager, the RN working in the facility provides direct resident care, evaluates outcomes and adjusts nursing care processes as indicated to ensure optimal resident care.(31%). + Application of Knowledge Performance Criteria - Demonstrates the ability to document in a clear, complete and organized manner, demonstrating skill in the nursing process. Demonstrates ability to provide direct resident care, evaluates and adjusts nursing care processes as directed by the Unit Manager or Director of Nursing. Demonstrates knowledge of complications associated with each category ofprocedures. Demonstrates knowledge of proper resources when unsure. Performs job duties within legal and professional regulations and guidelines. Identifies when help is required in providing resident care and seeks it appropriately, recognizing own limitations. + Communications Performance Criteria - Communicates pertinent information regarding resident progress and needs to appropriate nursing home staff. + Record Keeping Performance Criteria - Plans all work steps to ensure success; considers time, supplies and support required from other individuals and departments in setting up care plans and completing duties. 2) Administers and controls narcotics and medications in the facility as directed by nursing management.(20%). + Application of Knowledge Performance Criteria - Follows established protocols for narcotics and medications administration and resident observation. Monitors the medical condition of the resident and reports changes to the Unit Manager. Identifies emergency situations and directs immediate care. Complies with universal precautions to reduce occupational exposure. + Record Keeping Performance Criteria - Prepares accurate and timely documentation during all shifts of duty as required by guideline protocols. 3) Assists physicians with examinations, procedures and other processes related to direct resident care as directed. (5%). + Application of Knowledge Performance Criteria - Recognizes changes in resident condition and asks questions to clarify the situation. Reports changes to the Unit Manager. Maintains and improves resident optimal level of functioning and nursing rehabilitation. + Communications Performance Criteria - Explains procedures to residents thoroughly, provides assistance when necessary, monitors resident comfort and alleviates anxiety during procedures. + Collaboration Performance Criteria - Provides appropriate assistance to physicians during various medical procedures. + Record Keeping Performance Criteria - Accurately documents resident/family teaching and resident/family response to teaching. Accurately completes resident discharge summary according to departmental guidelines. 4) Assists physicians, co-workers and families as needed in time of stress or crisis. (5%). + Application of Knowledge Performance Criteria - Effectively identifies predictable problems and plans and provides resident care. + Communications Performance Criteria - Communicates clearly and tactfully with residents and their families. 5) Monitors and ensures accuracy of recording on resident medical records. Maintains all required reports, records, statistics as directed by the Unit Manager. (5%). + Application of Knowledge Performance Criteria - Reports accurate results to achieve the best possible resident care. Prepares accurate and timely documentation during all shifts of duty as required by guideline protocols. Follows facility protocol for admission documentation. Demonstrates skill in the nursing process. HIPAA Competency- Protects resident's privacy by insuring resident charts or items withresident data with protected health information (PHI) are not left unattended or left opened. + Management Performance Criteria - Ensures compliance with quality improvement policies and procedures. Contributes daily to updating the nursing care plan. Documents significant assessments to update resident records and charts. + Record Keeping Performance Criteria - Accurately documents resident/family teaching and resident/family response to teaching. Accurately completes resident discharge summary according to departmental guidelines. Completes logs and records promptly, recording all information that could be pertinent to others. Documents in a clear, complete and organized manner. Monitors staff and unit to ensure qualitystandards are maintained. 6) Contributes daily to updating the nursing care plan. Documents significant assessments to update resident records and charts. (5%). + Application of Knowledge Performance Criteria - Maintains a safe, comfortable and therapeutic environment for residents/families in accordance with nursing home standards. Exhibits expertise and skill in understanding, meeting and satisfying the specific needs and behavior of the resident in all age groups. Communicates with the resident effectively with or without language barriers or limitations. Demonstrates attitude of cooperation and professionalism. Promotes positive customer relations and maintains a professional appearance as per facility dress code. + Resident Relations Performance Criteria - Acts as an advocate of resident rights and assures resident and family understanding. Demonstrates mission facility by providing quality care, in a personal manner with total regard for individual human dignity. 7) The RN monitors and supervises the performance and follows the implementation of resident care by the CNA's on his/her unit. (5%). + Management Performance Criteria - Insures CNA Care Plan is followed at all times. Insures there is Range of Motion and Ambulation progress. Monitors breaks and lunches, insuring employees on the unit are absent from the unit only at designated times and duration. Provides feedback of CNA work performance to upper management. 8) Enhances professional growth and development within the Nursing Home through participation in educational programs, current literature, inservice meetings and workshops. (3%). + Application of Knowledge Performance Criteria - Performs successfully all skills assigned to this position's Competency Testing. Understands and demonstrates knowledge of the NYS Nurse Practice Act and the codes of the New York State Department of Health. Attends continuing education courses related to area of specialization to remain current in the field. 9) Attends all inservices and meetings as required. (2%). + Application of Knowledge Performance Criteria - Competency: Participates in drills and demonstrates knowledge of Plan for Department. Projects poise, authority, and a positive image. Wears appropriateclothing and accessories, giving proper attention to personal hygiene. Wears Facility ID Badge while onduty. Infection Control Competency: Demonstrates knowledge of and adheres to all infection controlprocedures including proper hand washing techniques, contact with blood spills and universal precautions. Attends all mandatory inservices. Attends department meetings as required and scheduled. The following duties may be reassigned: 10) Perform supervisory responsibilities on a as needed basis. (15%). + Application of Knowledge Performance Criteria - Monitors staff and unit to ensure quality standards are maintained. + Collaboration Performance Criteria - Cooperates with other personnel within the Nursing Home to achieve department objectives and maintain good employee relations, interdepartmental objectives, and infection control policies. 11) Orients, instructs and trains assigned personnel. (2%). + Application of Knowledge Performance Criteria - Ensures compliance with quality improvement policies and procedures. + Communications Performance Criteria - Communicates pertinent information regarding resident progress and needs to appropriate nursing home staff. + Collaboration Performance Criteria - Maintains harmonious relationship with unit and co-workers, providing support and assistance to co-workers to ensure quality resident care. 12) Ensures adequate stock of supplies and proper functions of equipment within Nursing Home.
