CMHC is an urban community mental health center with an exceptional 50-year tradition of serving individuals with serious mental illness. PURPOSE OF JOB CLASS (NATURE OF WORK) In a state hospital or facility works on a daily or hourly basis as a regular registered nurse, providing direct professional nursing care to a variety of individuals. EXAMPLES OF DUTIES + May participate as an active member of the multidisciplinary treatment team by providing substantial input regarding the patient’s response to treatment, recommending modifications in the overall plan of care; + Work under the supervision of a Head Nurse; + Complete comprehensive Initial Nursing Assessment for inpatients with acute psychiatric and addictive problems, assesses; + Demonstrate interviewing skills that facilitate accurate and comprehensive data collection and documentation in the 24Hr. Nursing Notes on patient you are assigned accurately assesses and documents patient’s response to pain interventions; + Foster a cooperative working relationship among staff members of all disciplines; + Effectively assumes charge responsibilities when designated; + May cover for Team Leader, may function in charge role supervising LPN’s and MHA’s. + Ensure that all clinical documentation progress notes are completed by MHA’s and LPN’s; + Identify and maintain safety of patients who are at imminent risk to self or others including; Risk to Fall, AWOL Risk, and medical instability; + Administer all medications in accordance with CMHC Standards of Practice and Nursing Policies and Procedures; + Collaborate with multidisciplinary Teams and assist in identifying patient problems and participate in team meetings; + Conduct inpatient groups such as community meetings, goals group, and orientation; + Participate in implementing health teaching interventions with patients and /or family members; + Attend shift reports and participates in nursing and multidisciplinary staff meetings; + Ensure that all documentation ensures patient safety by conducting environmental safety rounds and patient 15, 30 min checks, or 1:1 specials are assigned and policies and procedures are being followed. + Participate in the assessment and reassessment process by collection of data throughout the course of a shift hospitalization, documents changes in mental status evaluates and prescribes nursing treatment in crisis situation; + Make effective decisions and evaluates the results of action taken; + May take charge of Admissions and conducting patient assessments, including admission documentation ; + May be required to participate in restraints application, and assist in de-escalation of behavioral dyscontrol, inpatients, must attend and remain up to date on all DMHAS and CMHC mandatory training. + Conduct and teach patient debriefing following the use of seclusion and/or restraints in accordance with the Guidelines; + May be required to take clients to medical or clinical appointments and or community outings, and fresh air breaks; + Effectively communicate patient care needs ensures compliance with JCAHO and Medicare standards of nursing practice; + May perform all research related activities, blood draws, blood spinning, IVs, research protocol, treatment plans and participate all the training and competencies related to individual research protocol. + Performs other related duties as assigned. PREFERRED QUALIFICATIONS + Experience with psychiatric mental health nursing SPECIAL REQUIREMENTS + Incumbents in this class must possess and retain a current license or temporary permit to practice professional nursing in Connecticut. + Incumbents in this class may be required to be fluent in a foreign language or proficient in the use of American Sign Language in certain designated positions.
