Registered Nurse-Virtual Care Nurse Triage Call Center - Afternoons and Evenings

Schedule

Part-Time: 40 hours/biweekly

Selected candidates will interview onsite in York, PA. Employees in this position attend meetings/trainings onsite in York, PA. Must be willing to report onsite in York, PA as-needed with minimal notice.

Rotating six week standard schedule

Late afternoons and evenings shift with weekend commitment

General Summary

Under general supervision, performs primary triage screening to determine appropriate avenue for care of patients. Ensures care of patient by appropriate provider. Monitors access to care. Educates patients and families in home care techniques and the responsibilities of managed care.

Duties and Responsibilities

Essential Functions:

Obtains history and gathers triage information to determine appropriate avenue for care, identifies primary care physician. Uses critical thinking and assessment skills, as well as clinical judgement to determine the appropriate avenue of care. Impacts the system by keeping patients out of the Emergency Department unnecessarily and directing them to a lower level of care when appropriate. Educates patients and families on appropriate home care measuresRelays triage information and facilitates communication between the provider and the patient concerning patient disposition.Assists in the management of patient utilization of health care system and provides patient education relative to health care needs, use of community resources and responsibilities of membership in managed care.

Common Expectations:

Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and assists the clinical nurse to provide appropriate, age specific treatment.Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.Enhances professional growth and development through advanced/continuing education to maintain up-to-date knowledge, skills, and competencies.Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.Provides outstanding service to all customers using caring behaviors; fosters teamwork; and practices fiscal responsibility through improvement and innovation.

Qualifications

Minimum Education:

Diploma Program Required orAssociates Degree RequiredBachelors Degree Bachelors of Science, Nursing Preferred

Work Experience:

3 years Required5+ years Strongly Preferred (broad clinical practice)

Licenses:

Licensed Registered Nurse Upon Hire Required orRegistered Nurse Multi State License Upon Hire Required andBasic Life Support Upon Hire Required
