The Registered Nurse Triage role is a Registered Nurse who provides high quality telephonic triage and care coordination to patients across our network of community clinics. This role plays a vital role in assessing patient needs, prioritizing care, and ensuring timely referrals to appropriate health care services, all while delivering exceptional patient centered care in a virtual environment.
Responsibilities include:
Assess patient symptoms over the phone or in person to determine the appropriate level of care.
Provide timely recommendations, including referral to a higher level of care if necessary.
Respond to patient calls with professionalism and clinical expertise.
Manage in-basket communications, ensuring prompt resolution of patient inquiries and care needs.
Provide education on treatment plans, medications, and self-care strategies.
Communicate test results clearly and compassionately, answering any related questions.
Call in prescriptions as directed by the provider, ensuring accuracy and compliance with protocols.
Facilitate smooth transitions of care, including referrals to specialists or emergency care when appropriate.
Maintain thorough documentation of all patient interactions in the electronic medical record (EMR).
Qualifications include:
Current registered RN license in the state of Kansas and Missouri.
Minimum of three years of clinical nursing experience, with at least one year in triage, ambulatory care, or telehealth health settings preferred.
Strong clinical assessment, critical thinking, and decision-making skills.
