Unlock your potential at HCA Houston Healthcare Kingwood! **Job Summary and Qualifications** The Stroke Coordinator is fundamental to the development, implementation and evaluation of the stroke program directed towards the provision of quality services for patients who are experiencing stroke from a multi-disciplinary perspective throughout the continuum of care. **What you will do in this role:** + Analyzes and evaluates data and statistics to support the provision of quality services for patients experiencing stroke. + Manages and analyzes data relevant to critical pathway timeframes, evaluates patient outcomes, and identifies and intervenes on variances. + Develops evidence-based policies and procedures & performs gap analyses that explore and clarify areas for program improvement. + Constructing/presenting stroke system quality reports and proposing unique organization-specific quality goals and outcome measures. + Develops and champions implementation of action plans, in coordination with key stakeholders to address opportunities for improvement. + Works with leadership teams in developing and evaluating process and performance improvement activities as they relate to nursing and ancillary personnel. + Assures continuous quality improvement through data collection, analysis and process improvement projects. + Coordinates the development, implementation, review and revision of Stroke Program standards, policies, procedures and protocols. + Identify and refer appropriate cases to Quality for peer review process. + Develop evidence-based policies and procedures to support the stroke program as required in collaboration with the Stroke Medical Director. + Leads and participates in Quality Improvement processes and performance improvement projects. + Educational Responsibilities. + Develops and delivers acute stroke continuing education programs. **What qualifications you will need:** + (RN) Registered Nurse As a **full-service, 420+ bed acute care hospital** , HCA Houston Healthcare Kingwood (https://careers.hcahealthcare.com/pages/hca-houston-healthcare-kingwood) has provided the northeast Houston community with high-quality, cost-effective healthcare for over 25 years.
