Vancouver Coastal Health is looking for a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN), to join the Community Transition Care Team in Vancouver, BC.  Apply today to join our team! 


As a Registered Nurse (RN) / Registered Psychiatric Nurse (RPN) - Community Transition Care Team with VCH, you will:

Provide holistic nursing care in an interdisciplinary clinic setting to designated clients with a variety of acute and chronic illnesses/diseases, including mental health and substance use, communicable diseases, and/or general health issues.Use the principles of harm reduction, perform physical and mental health assessments, establishes, coordinates, reviews, and updates care plans, observes, monitors, and evaluates client progress and response to treatments, makes referrals and supports transition between services as needed.Provide education and counselling, and administers prescribed treatment, medication, and intravenous therapy in a safe and effective manner as required. Qualifications

Education & Experience

Current practising registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).One (1) year recent related clinical nursing experience providing care and/or treatment services to the designated client population through a harm reduction, holistic mental health and substance use lens, or an equivalent combination of education and experience.

Knowledge, Skills & Abilities

Working knowledge of community health nursing theory and practice within a client/family -centered approach.Working knowledge of the British Columbia College of Nurses and Midwives (BCCNM) standards of practice for nursing.Working knowledge of theory and established practices and procedures of professional nursing care for acute, chronic, and terminally ill clients.Working knowledge of facilitation, mediation, and conflict resolution techniques, and demonstrated analytical and problem-solving skills.Demonstrated case management, teaching/training, clinical support and consultation skills.Demonstrated skill in caring for a variety of parenteral therapy devices, including peripheral lVs, and central lines.Knowledge of the standards of practice for parenteral therapy.Demonstrated understanding of Trauma Informed Practice, Harm Reduction and Recovery Oriented Care.Broad knowledge of substance abuse and addictions treatment.Demonstrated knowledge of concurrent disorders, substance use, harm reduction strategies including assessment and intervention.Demonstrated ability to establish workload priorities in a dynamic environment.Demonstrated understanding of impact of social determinants of health on individuals, families and the community into care provided.Demonstrated knowledge and experience in providing culturally safe and competent care.Demonstrated knowledge of impact of colonization of individuals, families and the Downtown Eastside.Demonstrated ability to collaborate with site partners in a culturally safe manner, including community-based organizations, community members, and people actively using substances.Demonstrated ability to deal effectively with conflict situations.Demonstrated knowledge of substance use treatment, services and community resources.Demonstrated ability to communicate effectively, both verbally and in writing.Ability to provide physical, psychological and educationally supportive care and counseling.Ability to work effectively in an interdisciplinary team environment.Demonstrated ability to perform emergency procedures, including overdose response and CPR techniques.Ability to prioritize, work independently, and use a cooperative approach to program delivery.Ability to carry out the physical demands of the job.
