SummaryUnder the general direction of nursing leadership and with provider collaboration and guidance the Registered Nurse utilizes evidence based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments HODs and Medical Group in a variety of clinical settings This position facilitates care for a varied population of patients which includes nursing standards of practice to meet the physical emotional spiritual cultural and educational needs of the patient and familyProvides nursing within a non acute setting in collaboration with inter disciplinary teams and provides according to physical emotional spiritual cultural and educational needs of the patient and familyCoordinates and provides holistic patient centered careUtilizes evidence based practice in accordance with the department andor medical group policies and procedures to ensure safe quality care and customer serviceIncorporates the four components of the Professional Practice Model of Nursing into their daily practice Excel Empower Advocate and EngageDemonstrates and actively supports research utilization through inquiring about evidence based practicesEvaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and familyIdentifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patientsMaintains confidentiality of all patient informationSupport two clinical therapeutic groups dailyFacilitate activity group dailyBachelorrsquos degree in Nursing from an accredited School of NursingSuccessful completion of nurse residency if applicableOne year of experience with a minimum of hours of clinical experience as a licensed RN years preferredRegistered Nurse State of CTCertified BLS Provider credentialed from the American Heart Association AHAAdditional certifications in specific specialties are preferred and
