and accountability for utilizing the nursing process in the prevention and treatment of acute and chronic wounds ostomy and continence care Functions in roles of direct expert care clinical resource consultant staff and patient educator Applies the nursing process within the context of the organizations Framework for Professional Practice Collaboratively manages transitions across the continuum of care to assist patients or populations in achieving or maintaining an optimal level of health and achieving best outcome Recognized as professional role model mentor and clinical resource Reports to Program Manager Inpatient Wound TeamJob Responsibilitiesbull Provides direct care to patients with complex acute and chronic wounds urinary and fecal diversions stoma care fistulas percutaneous tubes pressure ulcers lower extremity wounds surgical wounds incontinencemoisture associated dermal conditions and othersbull Provides collaboration and consultation to the health care team in developing and implementing plans used in the identification and management of patients with potential or actual alteration in skin integrity and stoma fistula carebull Provides appropriate education to patients families caregivers and staff regarding wound and skin care ostomy care percutaneous tubes and draining woundfistulae managementbull Evaluates patient response to treatment in the progress of wound healing ostomy self care skills training and other outcomes and adjusts plan of care as indicatedbull Uses protocols and evidence based guidelines for appropriate and cost effective use of
