Registered Nurse (RN) - Home Health Infusion Coordinator, Charlottesville

UVA Continuum Home Health Care, the home health and home infusion division of UVA Health, has been providing exceptional in-home care to the community for over 30 years. We are currently seeking an experienced and highly skilled Registered Nurse – Infusion Coordinator to join our interdisciplinary care team. This advanced role requires a strong clinical background, excellent decision-making skills, and demonstrated leadership. The Infusion Coordinator plays a critical part in ensuring timely patient progression of infusion patients from hospital to home, acting as the first point of contact for new home health referrals and clinical coordination, reviewing clinical documentation and referrals to ensure appropriateness and readiness for home infusion services, coordinating with physicians, hospital discharge planners and the interdisciplinary team to develop individualized plans of care. The Infusion Coordinator is the front-line resource for patient and nursing staff questions. The infusion coordinator supports quality outcomes and continuity of care through proactive communication and planning.

MINIMUM REQUIREMENTS

Education: Graduate of an accredited nursing program required. Bachelor of Science in Nursing required within 5 years of hire.

Experience: 1 year of Relevant Experience

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia required. Valid Virginia Driver’s License required. American Heart Association (AHA) Health Care Provider BLS certification required.

PHYSICAL DEMANDS

Job may require sitting for prolonged periods, frequently standing, traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.

Position Compensation Range: $82,992.00 - $110,240.00 Annual
