Registered Nurse (RN) - Clinical Decision Unit

The Clinical Decision Unit (CDU) Nurse is responsible for providing high-quality, patient-centered care to individuals admitted to the Clinical Decision Unit for short-term observation or assessment. This role involves collaborating with interdisciplinary healthcare teams to manage patient care, monitor vital signs, administer treatments, and ensure appropriate documentation of patient interactions. The CDU nurse ensures patients are safely transitioned to either discharge, inpatient admission, or further specialized care based on clinical evaluations. Job Details Registered Nurse (RN) – CDU RN Patient Assessment and Monitoring: + Conduct thorough initial and ongoing assessments of patients admitted to the CDU. + Monitor and document vital signs, lab results, and patient symptoms to track changes in health status. + Administer medications, IV fluids, and treatments as prescribed by physicians. + Recognize early signs of deterioration and take appropriate action. Patient Care and Education: + Provide direct patient care, including comfort measures, emotional support, and patient education about procedures, medications, and follow-up care. + Educate patients and families on the clinical decision-making process, discharge planning, and potential outcomes. + Advocate for patient needs and preferences, ensuring their comfort and safety during their observation period. Clinical Decision-Making and Coordination: + Collaborate with physicians, nurse practitioners, and other healthcare providers to review patient conditions, diagnostic results, and treatment plans. + Participate in interdisciplinary rounds to discuss patient progress and make recommendations for care. + Ensure timely transitions of care, including arranging appropriate follow-up, referrals, or admission to inpatient services if necessary. Documentation and Reporting: + Maintain accurate and up-to-date patient records, including nursing assessments, medication administration, and changes in condition. + Communicate effectively with the care team and document changes in patient status, including any critical findings or interventions. + Ensure compliance with healthcare regulations, standards, and facility protocols. Quality Improvement and Safety: + Follow hospital and departmental protocols to ensure a safe, high-quality care environment. + Participate in quality improvement initiatives aimed at improving patient outcomes, safety, and patient satisfaction. + Adhere to infection control practices, safety guidelines, and regulatory requirements. Discharge Planning: + Assist in developing individualized discharge plans, ensuring patients understand discharge instructions and follow-up care. + Facilitate coordination with case management and social services for patients requiring additional resources post-discharge and placing post- discharge phone calls. Qualifications: + Strong clinical assessment skills and critical thinking. + Ability to work independently and as part of a multidisciplinary team. + Excellent communication, interpersonal, and organizational skills. + Proficiency in using electronic health records (EHR) systems. + Ability to prioritize and manage multiple tasks effectively. + 1+ year of RN experience working in telemetry preferred. + New Graduate Registered Nurses are welcome. + Associates in Nursing from an accredited school of nursing required. Bachelor’s Degree preferred. + Licensed and currently registered as a professional nurse in New York State. + BLS, ACLS required for all RN’s. Salary Range USD $55.13 - USD $64.36 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
