All while understanding that **together** we are even better. **Schedule:** Full Time **Shift** : Monday- Friday 8:45am- 5:15pm **Location:** Hospitals, homes, facilities in Arapahoe, Denver, Douglas, Elbert, Adams and Jefferson Counties **The role you'll contribute:** Coordinates and facilitates patient admissions or transfers according to patient acuity, bed availability, and required services. **The value you'll bring to the team:** + Facilitates patient care conferences with the multidisciplinary team as needed. + Monitors patient progress and adjusts care plans as necessary to ensure optimal outcomes. + Collaborates with nursing staff and multidisciplinary team members to place patients in appropriate levels of care to achieve desired outcomes. + Coordinates as liaison between physician requestors for consultants and specialty consultants. + Initiates care management processes including pre-registration and insurance verification. + Stays informed of changes in managed care and communicate procedural and financial impacts to admitting physicians during the reservation/scheduling process. **The expertise and experience you'll need to succeed:** **Minimum qualifications:** + Associate's of Nursing + Active American Heart Association BLS + Registered Nurse State Licensure and/or Compact State Licensure In Colorado This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
