Job Details Our Pediatric Home Care RN’s: + Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients. + Measure vital signs, Administer IVs, change dressings, clean wounds as needed + Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations. + Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload. + Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment. + Provide care to the most upmost standard Minimum Qualifications: + New York State RN license + Minimum of one year of recent hands on experience as a Registered Nurse. + Basic Life Support Certification + Valid New York State Driver’s License and reliable transportation Salary Range USD $95,000.00 - USD $125,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
