Key Responsibilities:

 Conduct thorough reviewof medical records to determine if potential candidates meet the specified criteria for kidney and/or pancreas transplants.Utilize your communication skills to connect with newly referred patients over the phone, assessing their medical history and candidacy for transplantation. Collaborate with a passionate,multi-disciplinary team, providing valuable insights and supportduring the referral phase, while ensuring all necessary information is meticulously gathered and documented.

QualificationsQualifications:Education:Graduate of an accredited nursing program requiredBSN requiredLicense/Certifications:Current, unrestricted California RN License requiredBLS from the American Heart Association requiredSpecialty or Transplant certification required or willing to obtain within one year from start dateExperience:A minimum of 3 years of Transplant or Acute Care experience requiredDemonstrated knowledge of case management requiredExperience in data management requiredICU experience preferredCedars-Sinai is an EEO employer.
