care of cardiac patients throughout the care continuum They will work closely with physicians in the Cardiac Cath lab to coordinate the patientrsquos care plan serving as a member of the multidisciplinary team functioning as a patient advocate educator and liaison between the patient and all other appropriate services after discharge The Nurse Navigator will coordinate and work collaboratively to ensure efficient transition of care for all referred cardiac patients The initial patient population will be confined to Hartford Hospital Medical Group Interventional Cardiology elective patients admitted to JB with plans to extend role as needed The role will also include collaboration with the office staff in Medical Office Building to identify high risk patients such as caregiver issues compliance problems high levels of anxiety or other special needs related to the procedure This role will work closely with the Cath Lab Manager to initiate collaboration with independently as well as maintaining an effective presence in a team settingbull Effective leadership skills with the ability to clearly communicate with tact and confidentialitybull Ability to adapt
