**Job Description:**
Responsible for specialty care management for assigned clinics/geographical areas and other appropriate referrals within the Intermountain system. Assess, plan, educate, and advocate for patients' health needs by collaborating with the patient/family, physician(s), other health care providers, national organizations, and community resources. Collaboratively develop and implement a plan of care that meets the patient's healthcare goals, promotes shared-decision making, optimizes allocation of resources, and facilitates patient engagement and self-care management across the care continuum.
As a Registered Nurse Case Manager, you will apply the Case Management Standards of Practice— **Assess, Plan, Facilitate, and Advocate** —to coordinate and optimize care for your assigned patient population. You will collaborate with interdisciplinary teams to ensure seamless, patient-centered care across the continuum.
**Key Responsibilities**
**Care Coordination & Case Management**
+ Apply case management principles to assess patient needs, develop care plans, and advocate for optimal outcomes.
+ Evaluate patient/family understanding of diagnoses and health goals; identify barriers to care and address them collaboratively.
+ Facilitate communication among patients, families, and healthcare teams to ensure continuity and quality of care.
+ Advocate for patients by navigating financial, logistical, psychosocial, and practical challenges using available resources.
**Interdisciplinary Collaboration**
+ Partner with physicians, patients/families, and care teams to establish goals and implement interventions.
+ Document all interactions and interventions; participate in team conferences and care planning sessions.
**Patient Acuity & Resource Navigation**
+ Determine appropriate acuity levels and guide patients through symptom management and care planning.
+ Educate and support patients across various care settings, ensuring timely access to services and resources.
**Professional Practice & Education**
+ Adhere to CMSA or specialty-specific standards of practice.
+ Maintain expert knowledge of internal and external resources.
+ Promote collaboration across the healthcare system and provide education to patients, families, and staff.
**Program Development & Leadership**
+ Contribute to regional and system-wide initiatives to improve care delivery.
+ Develop and revise educational and professional materials.
+ Perform additional duties as assigned.
**Data Tracking & Reporting**
+ Document activities in standardized databases or approved formats.
+ Support the refinement and development of tracking tools and metrics.
**Minimum Qualifications**
+ Current **Registered Nurse (RN)** license in the state of practice.
+ **Basic Life Support (BLS)** certification.
+ Minimum of **five years of direct clinical (patient care) experience** .
+ RNs hired or promoted into this role must obtain a **Bachelor of Science in Nursing (BSN)** within three years (not required for Nevada positions).
**Preferred Qualifications**
+ **BSN** from an accredited institution (degree verification required).
+ **Master’s degree** in Nursing or a related field.
+ Experience in **quantitative and qualitative analysis** and research methods.
+ Prior **leadership or supervisory experience** .
+ Minimum of **two years of clinical experience** with a specialty population.
**Physical Requirements:**
Ongoing need for employee to see and read information, labels, monitors, identify equipment and supplies, and be able to assess patient needs.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, etc.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
$44.42 - $65.70
